Please fill out the form with as much detail relevant to your tattoo as possible, with links to photos if necessary. It is very helpful to me if you only fill the form out once, so look over your submission carefully before hitting send. Again, please only submit once. We select clients based on the information provided, so be specific and considerate of other clients by sticking to your design choice to avoid scheduling conflicts. Once an appointment is secured, you will be asked to place a nonrefundable deposit, so if you change your mind about the design and we aren't able to accommodate you, you will lose your deposit. We schedule every 3 months so don't be discouraged if we aren't able to get you in! Thank you!!
